The Louisiana Department of Health and Hospitals (LDH) coordinates various statewide medicaid waiver programs to make services accessible to all individuals and families who qualify for Medicaid. Among these programs are elderly and disabled waiver programs and home and community based waivers. These cover services such as personalized in-home caregiving from our highly-trained caregivers.

Moore Care Caregiver Services acquired ISLAND Home Care in 2021, thus expanding our caregiving services into the Medicaid sector. ISLAND Home Care is currently accepting new Medicaid Waiver clients in the following parishes:

REGION 2: Ascension, East/West Baton Rouge, East/West Feliciana, Iberville, & Pointe Coupee

REGION 9: Livingston, St. Helena, St. Tammany, Tangipahoa, & Washington

The programs below are some of the Medicaid Waiver Programs and types that we honor:


Office of Adult Aging Services (OAAS)


Long-Term Personal Care Services (LTPCS):  LTPCS is a program administered by the LA Office of Aging and Adult Services for all Medicaid recipients ages 65 or older, or age 21+ with disabilities.  More information may be found here.


Community Choice Waiver (CCW):  CCW serves seniors and persons with adult-onset disabilities as an alternative to nursing facility care. More information may be found here.  


Adult Day Health Care Waiver (ADHC): This is a home and community-based alternative to nursing facility placement designed for individuals 65+ or ages 22-64 who meet certain criteria. More information may be found here.



Offices for Citizens with Developmental Disability (OCDD)


New Opportunities Waiver (NOW):  The mission of NOW is to utilize the principles of self-determination to supplement the family and/or community support while supporting dignity, quality of life, and security in the everyday lives of people while maintaining the Recipient in the community.  More information may be found here.


The Children's Choice waiver (CC): program provides services in the home and in the community to individuals 0 through 20 years of age, who currently live at home with their families or who will leave an institution to return home. More information may be found here.


Residential Options Waiver (ROW):  ROW was developed to assist persons with intellectual and developmental disabilities (ICD/IDD) as they transition from Intermediate Care Facilities to their own home in the community.  More information may be found here.
